Here are the result of the Ukrainian Futsal Awards 2004. A jury of futsal experts from Ukraine was asked to vote for three categories: best player "senior", best player "under 21" and best referee. According to the Awards result, the Shakhtar Donetsk player Ramis Mansurov was the best one in 2004. In the Under 21 players category the final victory went to the DSS Zaporozhye talent's Mikhail Romanov, while Yuri Rudinskiy (from Kharkov) was voted as the best futsal referee of the year.
